
The Mississippi Medicaid Population Health Demonstration Project is population level health program that seeks to reduce incidences of two prevalent medical phenomena among Medicaid beneficiaries: prediabetes leading to type II diabetes and preterm births. Through the Delta Medicaid Prediabetes Program, DHA partners with Mississippi Division of Medicaid and the Cerner Corporation to provide a litany of resources and services to reduce the pervasiveness of prediabetes among Medicaid recipients and the larger Delta community. Similarly, DHA’s Healthy Pregnancy Program connects participants with services and resources to reduce pre-term birth rates in the state.
