Leland Medical Clinic receives national recognition
(Stoneville) The Leland Medical Clinic, owned and operated by Delta Health Alliance (DHA), has been nationally recognized by NCQA (National Committee for Quality Assurance) as a Patient-Centered Medical Home – Level 2, an innovative approach to primary care. In addition to Leland Medical’s recognition, the clinic’s provider, NCQA, a private 501(c)(3) organization, has been a central figure for more than 25 years for driving improvement and transforming health care by developing quality standards.
The NCQA seal is a widely recognized symbol of quality. Organizations must first pass a rigorous, comprehensive review and must annually report on their performance. The PCMH seal is a reliable indicator that an organization is well-managed and delivers high quality care and service.
Karen Matthews, DHA’s chief executive officer, understands the significance of Leland Medical Clinic’s recognition. “This is a major accolade for Delta Health Alliance,” states Matthews, “the clinic is the only PCMH recognized clinic in the Mississippi Delta.” In addition to being the only Delta PCMH, Leland Medical joins 12 organizations state-wide that have also received this prestigious acknowledgement for providing quality patient care.
What is a “patient-centered medical home?” Daniel Thomas, Assistant Vice President IT/CIO and LMC director, explains it’s a model of care that strengthens the provider-patient relationship through coordinated care and a long-term healing relationship. The PCMH approach means that the entire LMC staff, as well as the patient, work together as a team by focusing on all aspects of the patient’s health using enhanced technology and chronic disease management. The team uses the Allscripts suite of products to enhance delivery of care by allowing our providers to spend more time examining the patient and less time on record keeping. The LMC staff can connect electronically to pharmacies and area hospitals and patients have real time access to their medical records via LMC’s online portal. Leland Medical’s care team consists of a nurse practitioner, nurse, medical assistant, registered dietician, community health worker, registration clerk, and office manager.
Leland Medical has more reasons to celebrate. The certified rural health clinic has begun $1.2M renovation and will hold a ground-breaking ceremony in the near future.
