The Delta BLUES (Better Living Utilizing Engagement Strategies) Diabetes Initiative is a collaborative program serving low-income residents in five rural Mississippi Delta counties. BLUES aims to reduce the morbidity and mortality of diabetes in rural, low-income, minority communities. These goals will be advanced through telehealth consultations, health education, and mentoring between endocrinology specialists and community-based clinicians at two rural health clinics in the heart of the Delta: the Leland Medical Clinic and the Leflore County Health Center. DHA will also deploy its Mobile Medical Clinic to serve rural patients residing outside of these population centers. Participants in this program will work with staff to develop customized goals with action plans to achieve those goals, and receive assistance enrolling in other social services including transportation access.
